Parent & Guardian Liability Waiver and Release Form
I, the undersigned parent or legal guardian, understand and acknowledge that working with and being around dogs always carries some level of inherent risk. I understand that participation in the Do Right’s Kids program and any related activities provided by Roxanne D’Onofrio and Doggie Do Right Dog Training is educational in nature and intended to help children learn safer behavior around dogs.
I further understand and agree that no guarantees or warranties can be made that a child will never be bitten, injured, frightened, or otherwise harmed by a dog, regardless of training, education, or supervision.
By enrolling and participating in this program, I voluntarily assume all risks associated with interactions between children and dogs. I agree that Roxanne D’Onofrio and Doggie Do Right Dog Training shall not be held liable or responsible for any accidents, injuries, damages, losses, bites, scratches, illnesses, emotional distress, or mishaps involving any child, dog, or participant that may occur before, during, or after participation in this course or related activities.
I understand that the best prevention and protection is active parental or adult supervision around all dogs at all times. I acknowledge that supervision of my child remains solely my responsibility.
